Request your records
Patients may request a copy of their medical records by completing a release-of-information form. Records can be released to you, another provider, or a third party (with your written authorization).
How to request
- Download or pick up a release form at any EPIC Health location.
- Complete and sign the form.
- Submit by email, fax, or in person.
- Receive your records within 30 days.
Transferring records to EPIC Health
Already a patient elsewhere? Ask your previous provider to forward your records — or our team will help coordinate the transfer at your first visit.
